• Weekly Estimated Pay: $2,500 – $2,630
• Location: Newton, MA (servicing Beverly, Marblehead, Salem, Swampscott areas)
• Assignment Duration: 13 weeks (May 3, 2026 – August 1, 2026)
• Shift: 8 Hour Days (08:00 AM – 04:30 PM)
• Hours per Week: 40 (Guaranteed)
• Weekend Rotation: Once every 6 weeks

Job Requirements (Qualifications)

• Active Registered Nurse (RN) license in Massachusetts
• Minimum 1 year of certified Home Care / Home Health experience
• Minimum 1 year of Case Management experience
• Ability to travel to patient homes as assigned across Newton and nearby communities
• Completion of required onboarding credentials (background check, health screenings, skills checklist, references)
• Comfortable maintaining accurate and timely electronic documentation (EHR) for home health visits

Responsibilities

• Assess patient conditions in the home and implement individualized homecare plans
• Provide patient and family education focused on disease management, medications, and home safety
• Deliver home health nursing care in accordance with organizational policy and community health standards
• Perform admissions, ongoing assessments, and case management duties for home health patients
• Maintain thorough, timely electronic documentation for all patient encounters
• Communicate and collaborate effectively with interdisciplinary teams, specialists, and community resources
• Participate in scheduled weekend rotations as required
