• Shift: 12-hour day shifts (3 shifts per week)
• Start Date: ASAP
• Work Type: On-site, Travel Contract (Travel RN)
• Schedule Note: Every 3rd weekend required

Job Requirements / Qualifications

• Minimum 2 years of bedside clinical experience as a Registered Nurse (RN)
• Active RN license in New Jersey (must be valid for assignment)
• BLS Certification (required prior to first shift)
• ACLS Certification (required prior to first shift)
• Experience in LTAC, sub-acute, skilled nursing, or similar settings preferred
• Preferred clinical skills: ventilator management, tracheostomy care, enteral tube feedings, IV therapy, wound care, and medication administration
• Strong assessment, clinical judgment, and communication skills for interdisciplinary team collaboration

Primary Responsibilities

• Perform comprehensive nursing assessments within 8 hours of patient admission using LTAC and acute care nursing standards
• Develop and implement individualized care plans in collaboration with patients, families, and interdisciplinary teams
• Provide direct patient care following the nursing process: assessment, planning, implementation, and evaluation
• Manage ventilator and tracheostomy-related care as indicated and participate in respiratory and rehabilitation programs
• Initiate discharge planning and patient/family teaching starting at admission to support safe transitions of care
• Document assessments, interventions, and patient responses accurately and in a timely manner to support continuity of care
• Promote patient and employee safety by adhering to facility policies, infection control, and safety protocols
• Supervise and provide direction to Licensed Practical Nurses (LPN), Certified Nursing Assistants (CNA), and orientee RNs when applicable
• Communicate changes in patient status to the interdisciplinary care team and attending providers to ensure timely interventions
• Assume charge responsibilities as requested following orientation and demonstrate leadership within the LTAC clinical setting
• Engage in ongoing professional development and continuing education to maintain LTAC clinical competencies
