Job Requirements / Qualifications
- Minimum 2 years of recent hospital pharmacy experience (required for hospital pharmacist roles)
- Successful completion of all educational requirements as mandated for state licensure
- Current State Pharmacist licensure required (or licensure eligible)
- BLS certification (if required by State)
- EPIC experience required (experience with electronic health record systems)
Responsibilities
- Maintain adequate drug supply and follow established procurement and inventory procedures for the hospital pharmacy
- Prepare, sterilize, and label parenteral medications and solutions according to policy and safety standards
- Fill and dispense medication orders, including STAT orders; ensure accuracy and timeliness in dispensing
- Ensure strict control and accountability for medications dispensed; maintain controlled substance documentation
- Document and record all dispensed drugs, maintain pharmacy records, and submit patient charges
- Conduct routine inspections of patient care units and pharmacy areas to ensure compliance and safety
- Participate in hospital formulary development, updates, and Pharmacy and Therapeutics Committee meetings
- Provide medication reconciliation for discharge medication lists and counsel patients and families on medication therapy
- Participate in antimicrobial stewardship programs, pharmacokinetic dosing management, and anticoagulation dosing management
- Monitor patient drug therapy regimens for contraindications, interactions, and therapeutic appropriateness
- Support quality assurance programs and continuous improvement initiatives within the hospital pharmacy
- Maintain confidentiality and uphold safety and regulatory standards in all pharmacy practice activities
