Job Details

• Weekly Estimated Pay: $2,320 - $2,440
• Assignment Length: 13 weeks
• Schedule: Monday – Friday, 8:00am – 5:00pm
• Guaranteed Hours: 40 hours per week
• Shift Duration: 8 hour days
• Territory: North territory (Seattle area) — Tukwila, WA
• Productivity: 6 visit points per day

Job Requirements

• Active Registered Nurse (RN) license in Washington
• Minimum 1 year of recent home health experience as a Home Health RN or homecare nurse
• OASIS assessment experience required
• Experience with HCHB (Homecare Homebase) EMR system
• Ability to work independently, manage a homecare caseload, and meet productivity standards

Responsibilities

• Provide skilled nursing care to patients in their homes, delivering high-quality and compassionate service as a Home Health Nurse
• Perform patient assessments, develop individualized care plans, and document accurately using OASIS and HCHB EMR
• Coordinate multidisciplinary care and communicate effectively with patients, families, and providers
• Maintain required productivity (6 visit points per day) while ensuring clinical quality and patient safety
• Participate in on-call or weekend coverage as needed (requirements TBD)
