Job Requirements

• Active LPN license in the state of New Hampshire (or compact eligible)
• Minimum 1 year of LPN experience in long-term care, skilled nursing, or a similar clinical setting
• Current BLS (Basic Life Support) certification
• Ability to work evening shifts and adapt to flexible scheduling
• Strong communication, teamwork, and resident-centered care skills

Responsibilities

• Provide compassionate resident care and support independence in a long-term care / SNF environment
• Administer medications and treatments safely and accurately as prescribed
• Monitor, assess, and document resident condition, vital signs, and care plans
• Assist residents with activities of daily living (ADLs) and promote quality of life
• Collaborate with interdisciplinary team members—RNs, CNAs, therapists, and providers—to ensure coordinated care
• Maintain a safe, clean environment and follow infection control practices
