Job Requirements

• Active Occupational Therapy (OT) license (state-specific licensure required for Everett, WA assignment)
• Minimum of 1 year of recent Home Health OT experience required (home health occupational therapist experience)
• Proficiency with HCHB (Homecare Homebase) documentation system
• Ability to travel within a 50+ mile radius of the Everett office
• Strong organizational, time management, and patient-centered care skills

Responsibilities

• Provide high-quality occupational therapy services to patients in their homes (home health OT care, functional rehabilitation)
• Complete clinical assessments, treatment plans, and therapeutic interventions focused on activities of daily living (ADLs) and functional independence in the home environment
• Travel to patient locations within a 50+ mile radius as needed for care coordination
• Meet productivity expectations (minimum 30 points per week) while maintaining clinical quality
• Complete all required documentation by 10:00 AM the following day using HCHB (accurate clinical documentation)
• Collaborate with interdisciplinary team members—nurses, PT, SLP, physicians—to ensure optimal patient outcomes
• Educate patients and families on therapy plans, home programs, equipment use, and discharge recommendations
