Qualifications & Job Requirements

• Active Registered Nurse (RN) license in the U.S. (state-specific license required prior to start)
• BLS Certification (required prior to first shift)
• ACLS Certification (required prior to first shift)
• Minimum 1 year of bedside clinical experience (2 years highly desired)
• Experience in LTAC, sub-acute, or similar post-acute/rehabilitation settings preferred
• Strong clinical assessment, documentation, and interdisciplinary communication skills

Key Responsibilities

• Perform comprehensive nursing assessments within 8 hours of patient admission
• Develop and implement individualized care plans in collaboration with patients, families, and interdisciplinary teams
• Provide direct patient care using the nursing process: assessment, planning, implementation, and evaluation
• Document assessments, interventions, and patient responses accurately and in a timely manner
• Participate in patient rehabilitation programs and initiate discharge planning from admission
• Communicate patient status changes to providers and ensure continuity of care during handoffs
• Supervise and provide direction to ancillary nursing personnel and support staff
• Promote patient and staff safety by adhering to facility policies, infection control, and best practices
• Contribute positively to unit teamwork, efficiency, and a patient-centered care environment
• Pursue professional growth opportunities and maintain required certifications
