Job Requirements
- Minimum two (2) years of bedside clinical experience (acute care, LTAC, or rehab experience valued)
- Active Registered Nurse (RN) license
- BLS Certification (required prior to first shift)
- ACLS Certification (required prior to first shift)
- Strong communication, care coordination, and teamwork skills
Responsibilities
- Perform comprehensive nursing assessments within 8 hours of patient admission in a long-term acute care setting
- Develop individualized care plans in collaboration with patients, families, and interdisciplinary teams to support rehabilitation and complex care needs
- Provide accurate and timely documentation of assessments, interventions, and patient responses to ensure continuity of care
- Participate actively in patient rehabilitation programs and deliver direct patient care focused on recovery and functional improvement
- Initiate discharge planning and patient teaching from the day of admission to support safe transitions to home or lower levels of care
- Communicate changes in patient status to care providers to ensure patient safety and quality outcomes
- Supervise and provide direction to ancillary nursing personnel and support staff as needed
- Promote patient and employee safety by adhering to hospital guidelines, infection control, and safety protocols
- Contribute positively to team building, unit efficiency, and a collaborative LTAC work environment
- Assume charge responsibility as requested after orientation and demonstrate leadership in patient care activities
- Engage in professional development and continuing education to maintain clinical competencies
