- Shift: 5x8-Hour Days (08:00 - 16:30)

- Weekly Estimated Pay: $2,170 – $2,330
- Mileage Reimbursement: IRS Standard

Qualifications & Requirements
- Licensed Occupational Therapist (OT) — Oregon license preferred; pending license accepted
- Minimum 1 year of skilled home health OT experience required (home health occupational therapy)
- BCLS (Basic Cardiac Life Support) certification required (AHA preferred)
- Experience with OASIS documentation and EPIC charting systems required
- Reliable transportation required (travel to patient homes; occasional radius up to 50+ miles)
- Primary COVID-19 vaccination series plus one booster required
- Ability to provide two supervisory references covering at least 1 year of employment within the last three years
- Comfortable working independently and managing a caseload up to 6 patients per day (30 productivity points standard)
- Strong clinical assessment, care planning, and patient education skills; experience with ADL training, adaptive equipment, and home safety evaluations preferred
Responsibilities
- Deliver skilled occupational therapy services to patients in their homes throughout Medford and surrounding areas
- Perform comprehensive patient assessments and develop individualized care plans focused on functional independence
- Provide therapeutic interventions including ADL retraining, adaptive equipment recommendations, therapeutic exercise, and home safety/fall prevention
- Document care accurately and timely using EPIC/OASIS in accordance with facility and regulatory standards
- Collaborate with interdisciplinary team members, caregivers, and family to optimize patient outcomes
- Educate patients and families on therapy goals, home programs, and safe use of adaptive devices
- Adhere to infection control, safety protocols, and professional standards for home health practice
