Required Qualifications
- Active Occupational Therapist (OT) license in Colorado (licensed occupational therapist - Colorado)
- Minimum 1 year of Occupational Therapy experience; home health experience preferred (home health OT preferred)
- Proficiency with OASIS documentation and Medicare home health requirements
- Ability to meet productivity requirement of 30 points per week
- Valid driver's license and reliable transportation for travel across El Paso, Teller, and Douglas counties
Primary Responsibilities
- Provide occupational therapy assessments and develop comprehensive plans of care, including discharge planning (hospital and home health OT)
- Evaluate patient environments for safety and accessibility; recommend adaptive equipment and home modifications
- Identify therapy diagnoses, set goals, implement interventions, and update care plans as needed
- Interpret and implement physician orders related to occupational therapy
- Coordinate interdisciplinary care with nurses, physical therapists, physicians, and case managers
- Complete OASIS and other regulatory documentation in compliance with agency and Medicare standards
- Educate patients and caregivers on diagnosis, medications, infection control, safety, and fall prevention
- Conduct medication reviews and provide related patient/family education
- Manage adaptive equipment and supplies to increase client independence and function
- Supervise occupational therapy assistants and home health aides as appropriate
- Maintain infection control standards, report incidents, and participate in QAPI activities
- Participate in staff meetings, patient care conferences, and in-services
Additional Skills & Preferred Experience
- Strong clinical judgment and time management for a mixed hospital/home health caseload
- Experience with electronic medical records and remote documentation
- Excellent communication and patient/caregiver education skills
