• Shift: 8:00 AM – 4:30 PM (8-hour days)
• Guaranteed Hours: 35 hours per week

Required Qualifications

• Active Occupational Therapist (OT) license in the state of New Mexico, or eligibility to obtain NM licensure
• Minimum 1 year of experience as an Occupational Therapist; experience in long term care, skilled nursing, or rehabilitation preferred
• Experience with ADLs, adaptive equipment, splinting, and patient-centered care plans
• Ability to work independently and collaboratively within a multidisciplinary healthcare team
• Strong communication, documentation, and organizational skills
• NM caregivers account required

Key Responsibilities

• Evaluate and assess residents to develop individualized OT treatment plans focused on function, safety, and independence
• Provide therapeutic interventions to improve mobility, ADLs, cognition, and adaptive strategies for geriatric and medically complex patients
• Recommend and train residents and families on adaptive equipment, home safety, and fall prevention
• Collaborate with nursing, PT, SLP, social work, and other rehabilitation staff to ensure coordinated care in the LTC/SNF setting
• Accurately document evaluations, progress notes, and plan of care in the medical record and adjust treatment based on outcomes
• Participate in care conferences, discharge planning, and quality improvement initiatives
