Qualifications (Home Health RN / Hospice RN)

• Current Registered Nurse (RN) license in the State of Montana
• BSN preferred
• Minimum 1 year recent acute care, home care, or hospice clinical experience (2 years preferred)
• Valid driver's license, appropriate auto insurance, and reliable transportation for in-home visits
• Clinical skills: venipuncture, urinary catheterization, wound care, and/or parenteral therapy
• Familiarity with OASIS documentation, home health agency standards, and hospice care practices preferred

Primary Responsibilities

• Provide skilled nursing care and comprehensive case management for assigned patients in their homes and at the in-patient Hospice Center
• Admit patients to services, conduct assessments, create care plans, implement interventions, and evaluate outcomes using the nursing process
• Deliver wound care, medication administration, venipuncture, catheter management, and parenteral therapy as needed
• Oversee and collaborate with Home Health/Hospice Aides and CNAs to ensure quality, coordinated care
• Maintain accurate, timely patient records in accordance with agency standards and OASIS requirements
• Participate in team scheduling, on-call rotation, quality improvement activities, and in-service training
• Educate patients and families to promote optimal health, comfort, and quality of life
