- Shift: Schedule varies; typical in-home RN schedule Mon–Fri 8am–5pm; in-patient Hospice schedule 3 rotating 12-hour shifts

Qualifications (Home Health RN / Hospice RN)
- Current Registered Nurse (RN) license in the State of Montana
- BSN preferred
- Minimum 1 year recent acute care, home care, or hospice clinical experience (2 years preferred)
- Valid driver's license, appropriate auto insurance, and reliable transportation for in-home visits
- Clinical skills: venipuncture, urinary catheterization, wound care, and/or parenteral therapy
- Familiarity with OASIS documentation, home health agency standards, and hospice care practices preferred
Primary Responsibilities
- Provide skilled nursing care and comprehensive case management for assigned patients in their homes and at the in-patient Hospice Center
- Admit patients to services, conduct assessments, create care plans, implement interventions, and evaluate outcomes using the nursing process
- Deliver wound care, medication administration, venipuncture, catheter management, and parenteral therapy as needed
- Oversee and collaborate with Home Health/Hospice Aides and CNAs to ensure quality, coordinated care
- Maintain accurate, timely patient records in accordance with agency standards and OASIS requirements
- Participate in team scheduling, on-call rotation, quality improvement activities, and in-service training
- Educate patients and families to promote optimal health, comfort, and quality of life
