• Location: Pendleton, IN (zip 46064) — Serving Grant, Blackford, and Delaware counties; Madison and Henry counties as overflow
• Setting: Hospital / Home Health (travel OT assignment)
• Schedule: Monday – Friday, 8-hour day shifts
• Hours per Week: 40
• Weekly Estimated Pay: $1,720 – $1,840

Job Requirements

• Active Occupational Therapy (OT) license — able to meet Indiana licensing requirements prior to start
• At least 1 year of home health OT experience within the past 3 years
• Experience with HCHB (Homecare Homebase) EMR preferred
• Strong communication, documentation, and organizational skills
• Skilled in assessments for ADLs, home safety evaluations, and adaptive equipment recommendations
• Ability to work independently in the field and collaboratively with a multidisciplinary hospital/home health team

Key Responsibilities

• Provide high-quality occupational therapy services in hospital and patient home settings as a travel OT
• Assess, plan, and implement individualized treatment plans focused on improving functional outcomes and independence
• Perform ADL training, home safety evaluations, and adaptive equipment assessments/recommendations
• Document patient progress and maintain accurate records using HCHB EMR
• Collaborate with physicians, nurses, case managers, and other rehabilitation professionals to coordinate care and support discharge planning
• Educate patients and families on therapy goals, home programs, and adaptive strategies to maximize independence
• Travel within the assigned territory to deliver home health services as needed
