Job Details – Hospice Travel Nurse / Home Health RN

• Weekly Estimated Pay: $2,200 – $2,310
• Assignment Duration: 13 weeks (March 9, 2026 – June 8, 2026)
• Setting: Hospice, Homecare — home health and palliative care
• Shift: Monday – Friday, 8:00 AM – 4:30 PM (8-hour days)
• Guaranteed Hours: 40 hours per week
• Coverage Area: Portland, OR and surrounding neighborhoods (zip 97232)
• Charting System: EPIC (remote client) — EPIC experience preferred

Required Qualifications — Hospice RN, Homecare Nursing

• Active Registered Nurse (RN) license in Oregon (Washington RN license a bonus)
• BLS (Basic Life Support) certification
• Minimum 1 year of hospice travel nursing or hospice/home health experience
• Familiarity with EPIC charting system preferred; strong clinical documentation skills
• Experience in palliative care, symptom management, and end-of-life care preferred

Key Responsibilities — Hospice Travel Nurse / Home Health RN

• Provide compassionate, patient-centered hospice care to adults in their homes
• Conduct thorough patient assessments and develop individualized hospice care plans
• Administer medications, manage symptoms, and perform skilled nursing treatments as ordered
• Coordinate care with interdisciplinary teams, including social work, chaplaincy, and physicians
• Educate patients and families on disease processes, symptom management, comfort care, and advance care planning
• Document visits and clinical notes accurately and timely using EPIC
• Support end-of-life care decisions and provide bereavement resources and family support
