• Shift: 8-hour days, Monday through Friday
• Guaranteed Hours: 40 hours per week
• Weekly Estimated Pay: $1,850 – $1,940 (competitive pay for travel home health RN)

Job Requirements

• Active Registered Nurse (RN) license; must be eligible to practice in Indiana
• At least 1 year of home health nursing experience within the past 3 years
• Proven experience with OASIS assessments and HCHB (Homecare Homebase) documentation
• Ability to travel between Indianapolis and Lafayette as scheduled (reliable transportation required for travel RN assignments)

Key Responsibilities

• Provide skilled, compassionate home health nursing care to patients in their homes as a Travel RN / Homecare Registered Nurse
• Perform comprehensive OASIS assessments and maintain accurate HCHB charting and documentation
• Meet productivity expectations (minimum 30 points per week) while maintaining high-quality patient care
• Collaborate with interdisciplinary teams to develop and implement individualized home care plans
• Educate patients and families on disease processes, medications, wound care, and self-management techniques
• Manage caseloads and coordinate travel between assigned locations (Indianapolis and Lafayette) to ensure continuity of care
