Job Details
- Location: Rutland, VT 05701 (Rutland, Vermont — Vermont home health jobs)
- Assignment Length: 13 weeks (April 27, 2026 – July 27, 2026)
- Schedule: 8:00am – 4:00pm, Sunday through Thursday (some on-call required)
- Guaranteed Hours: 37 hours per week
- Estimated Weekly Pay: $2,500 – $2,630
- Employment Type: Travel RN / Travel Nursing

Qualifications / Job Requirements
- Active RN license (Vermont or multistate compact nursing license)
- Minimum 1 year of recent Home Health RN experience
- OASIS documentation experience required (OASIS-C2 or current version)
- Current BLS certification (American Heart Association preferred)
- Strong clinical skills: IV therapy, wound VAC management, and complex wound care
- Excellent communication, organization, and time management skills
- Ability to work independently in the home setting and collaborate as part of an interdisciplinary team
Primary Responsibilities
- Assess patient and caregiver needs and deliver evidence-based home health nursing care
- Develop, implement, and communicate individualized plans of care with patient and caregiver involvement
- Serve as Case Manager for assigned patients, coordinating caseloads and visit schedules efficiently
- Complete OASIS documentation accurately and on time to ensure compliance and quality outcomes
- Provide skilled nursing interventions including IV management, wound VAC care, and advanced wound care techniques
- Maintain infection control standards and stay current on home health clinical competencies
- Collaborate with LPNs, therapists, social workers, and other team members to coordinate comprehensive care
- Educate patients and caregivers on medications, treatments, and self-care to support independence and safety
- Participate in interdisciplinary team meetings and case conferences to optimize patient outcomes
- Ensure accurate, up-to-date documentation and compliance with state, federal, and agency regulations
