Required Qualifications
- Active Registered Nurse (RN) license — Virginia or compact (Nurse Licensure Compact) accepted; pending Virginia license ok if obtained by Wednesday before start date
- Minimum 1 year recent adult home health and home infusion nursing experience
- Current BLS certification (American Heart Association preferred)
- Epic clinical documentation/charting experience required
- Previous travel nursing experience required
- Flu vaccine required (no exemptions)
- References required; Nursys verification required for submittal
- BSN preferred
Responsibilities — Home Health RN / Homecare Nurse Duties
- Provide comprehensive nursing care to adult and geriatric patients in home health settings, delivering patient-centered care and education
- Perform home infusion therapy and manage peripheral IVs, central lines, and implanted ports
- Conduct admission assessments, routine visits, recertifications, and skilled nursing evaluations (average 5 visits per day)
- Manage wound care, pressure ulcer prevention, and wound V.A.C. therapy
- Administer medications, including IV and subcutaneous infusions, and monitor responses
- Coordinate care with interdisciplinary team members, case managers, and home health aides
- Document clinical assessments, interventions, and patient education accurately and timely in Epic
- Educate patients and families on disease management, medication administration, and medical equipment use
- Respond to changes in patient condition and adjust care plans within scope of practice
- Flex and float within scope of practice as required by assignment
