Qualifications / Job Requirements
- Active Registered Nurse (RN) license (state of Virginia or compact RN license)
- BLS Certification (required prior to first shift)
- ACLS Certification (required prior to first shift)
- Minimum 2 years of bedside clinical experience
- PCU / Critical Care experience highly desired (LTAC, step-down, ICU experience preferred)
Responsibilities
- Perform comprehensive nursing assessments within 8 hours of patient admission and develop individualized care plans
- Collaborate with patients, families, and interdisciplinary teams to identify teaching needs, rehabilitation goals, and safe discharge plans
- Accurately document assessments, interventions, and patient responses in the electronic medical record
- Participate actively in patient rehabilitation programs and coordinated discharge planning for LTAC patients
- Communicate changes in patient status to providers and care team members to ensure continuity and patient safety
- Supervise and provide direction to LPNs, CNAs, and orientees; support a positive team environment
- Promote a safe environment by adhering to facility safety policies, infection control, and best nursing practices
- Engage in ongoing professional development and continuing education to maintain clinical competency
