Qualifications / Job Requirements

• Active Registered Nurse (RN) license (state of Virginia or compact RN license)
• BLS Certification (required prior to first shift)
• ACLS Certification (required prior to first shift)
• Minimum 2 years of bedside clinical experience
• PCU / Critical Care experience highly desired (LTAC, step-down, ICU experience preferred)

Responsibilities

• Perform comprehensive nursing assessments within 8 hours of patient admission and develop individualized care plans
• Collaborate with patients, families, and interdisciplinary teams to identify teaching needs, rehabilitation goals, and safe discharge plans
• Accurately document assessments, interventions, and patient responses in the electronic medical record
• Participate actively in patient rehabilitation programs and coordinated discharge planning for LTAC patients
• Communicate changes in patient status to providers and care team members to ensure continuity and patient safety
• Supervise and provide direction to LPNs, CNAs, and orientees; support a positive team environment
• Promote a safe environment by adhering to facility safety policies, infection control, and best nursing practices
• Engage in ongoing professional development and continuing education to maintain clinical competency
