Qualifications / Job Requirements

• Active RN license (compact privileges accepted where applicable) — Registered Nurse required
• Minimum 1 year of recent RN experience; home health experience or community nursing experience strongly preferred
• Associate's Degree in Nursing or higher (ADN, BSN acceptable)
• Current BLS/CPR certification (AHA preferred)
• Ability to work independently in the field and manage caseloads for home health patients
• Reliable transportation and valid driver's license for home visits

Primary Responsibilities (Homecare RN / Home Health RN)

• Provide skilled registered nursing care on a per-visit basis in home health and community settings
• Conduct comprehensive patient assessments and develop individualized care plans
• Perform skilled nursing interventions including wound care, medication management, catheter care, and IV/ostomy care as needed
• Deliver patient and caregiver education focused on disease management, medication adherence, and discharge planning
• Document visits and clinical care accurately in the electronic medical record (EMR) and maintain timely visit notes
• Communicate patient status, changes, and care coordination needs with physicians and the interdisciplinary home health team
• Follow agency policies, infection control standards, and state home health regulations
