Job Details – Travel Nurse / Home Health RN

• Location: Portland, OR 97219 (serving a 60-mile radius from Salem)
• Weekly Estimated Pay: $2,120 – $2,230
• Start Date: May 4, 2026
• End Date: August 31, 2026
• Duration: 17 weeks
• Shift: Days, 8:00 AM – 4:30 PM
• Hours per Week: 40 (guaranteed)
• On Call/Weekend Requirement: Yes
• Travel: Mileage reimbursed between patient visits

Key Qualifications – Registered Nurse (RN)

• Active RN license in the state of Oregon (required)
• Current CPR certification (BLS preferred)
• Valid driver's license and reliable transportation
• Associate's Degree in Nursing or higher
• Minimum 1 year of nursing experience required (home health experience preferred)
• Experience with wound care and HCHB EMR is a plus
• Strong clinical assessment, patient education, and care coordination skills

Responsibilities – Home Health Nursing / Community-Based Care

• Conduct Start of Care (SOC) visits, recertifications, evaluations, and Resumption of Care (ROC) visits
• Provide subsequent skilled nursing visits and ongoing home health nursing care in patients' homes
• Deliver compassionate, patient-centered care to a diverse community across Portland and surrounding areas
• Document all care accurately and promptly using electronic medical records (EMR/HCHB)
• Collaborate with interdisciplinary teams, case managers, and physicians to ensure optimal patient outcomes
• Participate in on-call and weekend rotations as required
