- Shift: Friday, Saturday, Sunday, Monday (8:00am – 6:00pm)
- Shift Duration: 10-hour days
- Guaranteed Hours: 40 hours per week
- Start Date: April 27, 2026
- End Date: June 21, 2026
Required Qualifications
- Minimum 1 year RN experience (Home Health & Hospice experience preferred)
- Active Registered Nurse (RN) license in Pennsylvania
- Valid Driver's License and reliable transportation
- Valid auto insurance
- BLS (CPR) certification from AHA
- Experience with HCHB (Homecare Homebase) and hospice care required
Key Responsibilities – Home Health RN / Hospice RN
- Conduct admission, routine, PRN, and death visits (approximately 6–8 visits per day)
- Assess patient health status and develop and implement individualized home health and hospice care plans
- Provide case management, health maintenance, and disease prevention education to patients and families
- Coordinate interdisciplinary communications among physicians, therapists, social workers, and other care team members
- Manage medication ordering, review, monitoring, and disposal according to physician orders
- Document assessments, care plans, progress notes, and visit information accurately in HCHB
- Maintain clinical equipment and supplies and adhere to infection control best practices
- Direct and supervise LPNs/VNs and unlicensed staff as needed; provide training and annual recertification on health and safety topics
- Schedule and follow up on appointments, referrals, and clinical findings to ensure continuity of care
