Job Details

• Weekly Estimated Pay: $2,120 - $2,230
• Contract Duration: 13 weeks (April 20, 2026 – July 19, 2026)
• Shift: Monday to Friday, 8:00 AM – 4:30 PM (no on-call or weekend requirements)
• Hours per Week: 40 guaranteed
• Travel: Mileage reimbursed between patient visits
• Dress Code: Any solid color scrubs

Home Health RN Responsibilities

• Perform RN OASIS Start of Care assessments and follow standardized OASIS documentation
• Manage RN discharge planning and recertification processes for home health patients
• Provide medication setup and safe medication management in the home setting
• Deliver skilled wound care, dressing changes, and assessment of wound healing
• Meet productivity expectations (30 points) while maintaining high-quality, patient-centered care
• Coordinate care across interdisciplinary teams and communicate effectively with patients and families

Required Qualifications

• Active Registered Nurse (RN) license
• Associate's Degree in Nursing (ADN) or higher
• Minimum 1 year of home health experience or comparable community-based nursing experience
• Experience with HCHB (Homecare Homebase) or similar electronic documentation systems
• Strong communication skills, reliable transportation, and a positive, patient-focused attitude
