• Weekly Estimated Pay: $2,120 - $2,230
• Location: Wilson, NC 27893; serving Nash County, Edgecombe County & Wilson County (North Carolina home health)
• Employment Type: Travel Contract (Travel RN / Travel Nursing Job)
• Duration: 18 weeks (April 27, 2026 – August 31, 2026)
• Hours: 40 hours per week (Guaranteed 36 hours)
• Shift: Days, 8:00 am – 4:30 pm
• On Call/Weekend Requirement: None
• Dress Code: Scrubs (no particular color)
• Mileage Reimbursement: Yes, between patient visits

Job Requirements (Home Health RN / RN Case Manager)

• Active Registered Nurse (RN) license in North Carolina
• Associate's Degree in Nursing (ADN) or higher
• Minimum 1 year of nursing experience; homecare/home health experience preferred
• Strong clinical assessment, care planning and case management skills
• Experience with electronic home health charting (HCHB/Homecare Homebase experience is a plus)
• Current professional certification required (e.g., BLS or other relevant certification)

Responsibilities (Home Health Nursing / Case Management)

• Serve as RN Case Manager for home health patients across Wilson, Nash and Edgecombe counties
• Coordinate and deliver patient-centered home health care and skill-based nursing services
• Conduct comprehensive in-home assessments and develop individualized care plans
• Monitor patient progress, perform clinical interventions, and revise care plans as needed
• Collaborate with interdisciplinary team members and communicate effectively with patients and families
• Maintain accurate, timely documentation and meet productivity expectations for home health visits
