Job Requirements (Registered Nurse / RN)
- Active Registered Nurse (RN) license in Tennessee or a current compact state license
- BLS (Basic Life Support) certification – required prior to first shift
- ACLS (Advanced Cardiovascular Life Support) certification – required prior to first shift
- Minimum of 2 years of bedside clinical nursing experience; critical care or ICU experience highly desired
- Experience with LTAC patient population preferred (ventilator management, tracheostomy care, wound care)
- Experience with electronic medical records (EMR) such as Epic or Cerner preferred
Responsibilities
- Perform comprehensive nursing assessments on all LTAC patients within 8 hours of admission and develop individualized care plans
- Provide complex, skilled nursing care for long-term acute care patients, including monitoring ventilators and managing tracheostomies when applicable
- Identify teaching needs and goals for patients and families; provide education and collaborate with interdisciplinary teams
- Document assessments, interventions, and patient responses accurately and promptly in the EMR
- Participate in patient rehabilitation programs, care coordination, and discharge planning to support successful transitions of care
- Communicate changes in patient status to the care team to ensure continuity, safety, and high-quality outcomes
- Supervise and provide direction to Licensed Practical Nurses (LPNs) and Certified Nursing Assistants (CNAs)
- Promote and maintain a safe environment by adhering to hospital safety policies, infection control, and LTAC protocols
- Contribute to a positive team environment, engage in ongoing professional development, and support quality improvement initiatives
