Job Requirements / Qualifications

  • Active Registered Nurse (RN) license (state of assignment required)
  • BLS Certification (required prior to first shift)
  • ACLS Certification (required prior to first shift)
  • Minimum 1 year of bedside clinical experience (2 years highly desired)
  • Experience in LTAC, long-term acute care, or similar acute care/rehabilitation settings preferred
  • Experience with ventilator, trach care, wound management, or complex case management is a plus for LTAC RNs

Key Responsibilities

  • Perform comprehensive nursing assessments within 8 hours of admission and develop individualized LTAC care plans
  • Collaborate with interdisciplinary teams (physicians, therapists, case managers) and families to set rehabilitation goals
  • Document assessments, interventions, and patient responses accurately and in a timely manner in the EMR
  • Participate in patient rehabilitation programs focusing on education, discharge planning, and quality outcomes
  • Initiate and coordinate discharge planning, patient teaching, and caregiver education from admission
  • Report and communicate changes in patient status to appropriate care providers to ensure continuity and safety
  • Supervise and provide direction to ancillary nursing personnel and assist with unit workflow
  • Promote patient and employee safety by following hospital policies, infection control, and LTAC best practices
  • Assume charge nurse responsibilities as requested and support unit efficiency and team building
  • Pursue professional growth through continuing education and on-the-job learning opportunities
