Job Details

• Position Type: Travel contract-to-hire nursing job
• Weekly Estimated Pay: $1,760 - $1,850
• Schedule: Friday through Monday, 8:00 AM to 6:00 PM
• Shift Duration: 10-hour day shifts
• Hours per Week: 40
• Contract Length: 13 weeks
• Setting: Home health and hospice community care
• Service Area: York and Lancaster counties, PA
• Productivity Goal: 6 to 8 visits per day

Job Requirements

• Active Pennsylvania RN license
• Hospice experience required
• Home Care Home Base (HCHB) experience required
• Valid driver’s license and current auto insurance
• Current BLS/CPR certification
• Strong communication, organization, and clinical documentation skills
• Ability to work independently while coordinating with the broader clinical team

Responsibilities

• Complete admission, routine, PRN, and end-of-life home health and hospice visits
• Assess patient needs, implement individualized care plans, and document care accurately
• Coordinate with physicians, inpatient nursing, and discharge teams as needed
• Monitor medications and support safe medication administration practices
• Communicate changes in patient condition promptly and professionally
• Maintain timely nursing documentation to support continuity of care
• Educate unlicensed staff on patient-specific health, safety, and disease prevention needs
• Provide compassionate, high-quality patient care to support patients and families in the community
