Job Details
- Position Type: Travel contract-to-hire nursing job
- Weekly Estimated Pay: $1,760 - $1,850
- Schedule: Friday through Monday, 8:00 AM to 6:00 PM
- Shift Duration: 10-hour day shifts
- Hours per Week: 40
- Contract Length: 13 weeks
- Setting: Home health and hospice community care
- Service Area: York and Lancaster counties, PA
- Productivity Goal: 6 to 8 visits per day
Job Requirements
- Active Pennsylvania RN license
- Hospice experience required
- Home Care Home Base (HCHB) experience required
- Valid driver’s license and current auto insurance
- Current BLS/CPR certification
- Strong communication, organization, and clinical documentation skills
- Ability to work independently while coordinating with the broader clinical team
Responsibilities
- Complete admission, routine, PRN, and end-of-life home health and hospice visits
- Assess patient needs, implement individualized care plans, and document care accurately
- Coordinate with physicians, inpatient nursing, and discharge teams as needed
- Monitor medications and support safe medication administration practices
- Communicate changes in patient condition promptly and professionally
- Maintain timely nursing documentation to support continuity of care
- Educate unlicensed staff on patient-specific health, safety, and disease prevention needs
- Provide compassionate, high-quality patient care to support patients and families in the community
