• Work Schedule: Monday through Friday, 8:00 AM to 4:30 PM
• Hours per Week: 40
• On-Call Requirement: Rotating weeknight and weekend coverage
• Productivity Expectation: 30 points

Job Requirements

• Active Registered Nurse (RN) license
• Minimum of 1 year of nursing experience; home health experience preferred
• Associate degree in Nursing or higher from an accredited program
• Current CPR certification
• Strong independent assessment and clinical decision-making skills
• Clean driving record and ability to travel to patient homes
• Experience with OASIS documentation preferred
• Comfort using EMR systems, including home health documentation platforms
• Background in wound care and IV medication administration preferred

Responsibilities

• Provide skilled nursing care directly in patients' homes
• Conduct thorough patient assessments and ongoing monitoring
• Administer medications and manage treatment plans as ordered
• Perform wound care and other skilled nursing procedures
• Complete accurate regulatory documentation, including OASIS assessments
• Educate patients and families on care plans, medications, and symptom management
• Coordinate care with interdisciplinary team members to support continuity of care
• Help reduce avoidable hospital readmissions through proactive clinical support
