Guaranteed Hours: 36 per week
Hours Per Week: 40
Shift Duration: 8 hours
Schedule: Monday-Friday, 8:00 AM - 4:30 PM
On-Call / Weekend Rotation: Required
Travel: Local travel between patient visits within the assigned service area

Job Requirements

Active Registered Nurse (RN) license
Minimum of 1 year of nursing experience required
Home health, homecare, or home health nursing experience preferred
Associate degree required
Strong communication, organization, and time-management skills
Ability to work independently in a home-based care environment
HCHB experience is a plus
Comfortable with a travel assignment and on-call/weekend rotation

Responsibilities

Provide skilled nursing care in the home setting
Complete patient assessments and deliver care according to the plan of care
Monitor patient status and communicate changes to the care team as needed
Document visits accurately and in a timely manner
Support safe, high-quality patient outcomes through professional nursing practice
Travel to patient homes within the assigned area for scheduled visits
Participate in on-call and weekend coverage as scheduled
