• Weekly Hours: 36 hours per week
• Shift: Day shifts, Monday through Friday, with one-in-four call rotation for nights, weekends, and holidays
• Shift Duration: 12-hour rotating shifts
• Guaranteed Hours: 32 hours
• Weekly Estimated Pay: $1,800-$1,890

Job Requirements

• Active Registered Nurse (RN) license in good standing
• Minimum of 2 years of home health nursing experience
• Experience completing OASIS assessments and care planning for in-home patients
• Strong clinical judgment, organization, and communication skills
• Ability to work independently while coordinating effectively with the care team
• Experience with Epic is a plus

Responsibilities

• Provide skilled nursing care to patients in their homes
• Complete comprehensive OASIS assessments and develop individualized care plans
• Monitor patient progress and communicate updates to the interdisciplinary care team
• Educate patients and families on care needs, safety, medication guidance, and follow-up instructions
• Participate in the department call rotation as scheduled, including nights, weekends, and holidays
• Maintain accurate, timely, and compliant clinical documentation
