• Mileage Reimbursement: Yes, between visits and from the first patient to the last patient

Job Requirements

• Active Texas RN license
• Associate degree in Nursing or higher
• At least 1 year of Home Health Field RN experience
• Experience with OASIS documentation preferred
• HCHB experience preferred
• Dependable, organized, and comfortable working independently in the field
• Strong communication, assessment, and patient education skills

Responsibilities

• Provide skilled nursing care to home health and hospice patients in their homes
• Complete comprehensive field assessments and routine patient visits
• Develop, follow, and update individualized care plans
• Document care accurately and in a timely manner according to agency standards
• Coordinate with interdisciplinary care teams to support positive patient outcomes
• Educate patients and families on care needs, safety, and treatment plans
• Maintain productivity expectations while delivering compassionate, patient-centered care
