• Shift Duration: 8 hours
• Schedule: Monday through Friday, 8:00 AM to 5:00 PM, with on-call coverage and weekends as needed
• Estimated Weekly Pay: $1,760 - $1,850

Job Requirements

• Active West Virginia Registered Nurse (RN) license
• Minimum of 1 year of nursing experience required
• Home health RN experience strongly preferred
• Experience with HCHB and OASIS documentation preferred
• Strong assessment, documentation, and care coordination skills
• Ability to work independently and manage multiple patient needs
• Reliable transportation for travel within assigned counties

Responsibilities

• Assess, plan, implement, and evaluate nursing care for patients in the home or community setting
• Support health maintenance, disease prevention, and case management activities
• Coordinate care with physicians, providers, and other healthcare professionals
• Monitor medications, ensure accurate clinical documentation, and report changes in patient condition promptly
• Complete daily, monthly, quarterly, and annual nursing documentation as required
• Educate and support staff on health, safety, and person-specific care needs
• Maintain timely communication regarding patient status, appointments, and hospitalizations
• Travel within assigned service areas to provide quality patient-centered care
