This 14-week travel nursing assignment is ideal for a motivated Registered Nurse who enjoys patient-centered care, autonomy, and variety in a home health and homecare setting. The schedule is Monday through Friday, 8:00 AM to 4:30 PM, with rotating weekends approximately every 5 weeks. Mileage reimbursement between patient visits is available, and the role includes 40 guaranteed hours per week.

Job Requirements

• Active Massachusetts RN license
• Bachelor's degree required
• Ability to work autonomously in a home care environment
• Quick learner with the ability to onboard fast
• Preferred: nursing home care experience with emphasis on post acute patients and assisted living patients
• Strong assessment, communication, documentation, and care coordination skills
• Comfort with OASIS documentation and medication reconciliation

Responsibilities

• Develop, implement, and revise individualized plans of care in collaboration with physicians and the care team
• Provide high-quality clinical services in the home while following infection control standards and plan-of-care guidelines
• Complete comprehensive assessments, including OASIS documentation and medication reconciliation, accurately and on time
• Document patient visits thoroughly per policy and payer requirements
• Supervise and support home health aides and LPN/LVNs as appropriate, including supervisory visits
• Coordinate care across the interdisciplinary team to support strong patient outcomes
