Job Requirements
- Active Michigan Registered Nurse license required by start date
- Minimum of 1 year of nursing experience required
- Adult home health experience required
- Epic experience required
- BLS certification required
- Must be comfortable with travel nursing assignments
- Must not be a first-time traveler
- Strong knowledge of Medicare home care guidelines and OASIS documentation
- High school level education or equivalent required
- Proficiency with skilled home care procedures, including PICC lines, ports, tracheostomy care, PleurX management, and patient education
Responsibilities
- Provide skilled nursing care to adult and geriatric patients in the home setting
- Complete comprehensive assessments, start-of-care visits, recertifications, and resumption-of-care visits
- Perform accurate clinical documentation, including OASIS assessments and ongoing charting in Epic
- Support medication, wound, catheter, feeding tube, and IV/central line care as needed
- Deliver patient and family education to promote safe, effective care at home
- Manage visits independently while maintaining strong clinical judgment and organization
- Collaborate with interdisciplinary partners to support high-quality patient outcomes
- Follow home care protocols and Medicare guidelines to ensure compliant, patient-centered care
