Job Requirements

• Active Michigan Registered Nurse license required by start date
• Minimum of 1 year of nursing experience required
• Adult home health experience required
• Epic experience required
• BLS certification required
• Must be comfortable with travel nursing assignments
• Must not be a first-time traveler
• Strong knowledge of Medicare home care guidelines and OASIS documentation
• High school level education or equivalent required
• Proficiency with skilled home care procedures, including PICC lines, ports, tracheostomy care, PleurX management, and patient education

Responsibilities

• Provide skilled nursing care to adult and geriatric patients in the home setting
• Complete comprehensive assessments, start-of-care visits, recertifications, and resumption-of-care visits
• Perform accurate clinical documentation, including OASIS assessments and ongoing charting in Epic
• Support medication, wound, catheter, feeding tube, and IV/central line care as needed
• Deliver patient and family education to promote safe, effective care at home
• Manage visits independently while maintaining strong clinical judgment and organization
• Collaborate with interdisciplinary partners to support high-quality patient outcomes
• Follow home care protocols and Medicare guidelines to ensure compliant, patient-centered care
