• Shift: 12-hour day shifts
• Hours per Week: 36

• Float: May float within the Milwaukee market as needed

Job Requirements

• Active Registered Nurse (RN) license
• Minimum of 2 years of bedside clinical experience
• BLS certification required prior to first shift
• ACLS certification required prior to first shift
• Strong assessment, documentation, and care coordination skills
• Ability to work effectively in a fast-paced, team-oriented LTAC healthcare environment
• Commitment to patient safety, professionalism, and high-quality nursing care

Responsibilities

• Complete nursing assessments for assigned patients within 8 hours of admission
• Develop individualized plans of care in collaboration with patients, families, and the interdisciplinary team
• Provide timely updates for team conferences and re-evaluations
• Document assessments, interventions, and patient responses accurately and thoroughly in the medical record
• Support patient rehabilitation and recovery through direct, compassionate clinical care
• Initiate discharge planning and patient teaching early in the admission process
• Communicate changes in patient status to the appropriate care providers to ensure continuity of care
• Supervise and direct ancillary nursing personnel as appropriate
• Promote a safe environment for patients, staff, and visitors
• Contribute to a positive team culture and assist with unit needs as required
• Assume charge responsibilities after orientation, if requested
• Continue professional growth through ongoing learning and skill development
