Job Requirements

• Active New York State RN license
• Minimum of 2 years of long-term care or long-term acute care experience
• BLS certification from the American Heart Association
• Telemetry experience preferred
• Strong communication, flexibility, and delegation skills
• Ability to work collaboratively in a resident-centered care environment

Responsibilities

• Provide direct patient care to geriatric long-term care residents
• Perform nursing assessments and document interventions accurately and thoroughly
• Implement and evaluate individualized nursing care plans
• Administer medications safely and effectively in accordance with clinical standards
• Provide oversight and support to LPN and CNA team members
• Deliver wound care and ongoing nursing support for residents with chronic conditions
• Maintain clear communication with the care team to support high-quality resident outcomes
