- Schedule: Monday-Friday, 8:00 AM-4:00 PM
- Shift Duration: 8-hour days
- Additional Coverage: On-call, weekend rotation, and holidays required
- Weekly Estimated Pay: $2,500-$2,630

Job Requirements
- Active Registered Nurse (RN) license
- At least 1 year of nursing experience required; home health or hospice RN experience strongly preferred
- OASIS documentation experience required
- Experience with case management and patient-centered care planning
- Strong knowledge of home health workflows, clinical documentation standards, and compliance requirements
- Comfortable working independently in the field and managing a patient caseload
- Ability to support on-call hospice coverage as needed
- Experience with wound care, IV therapy, wound VACs, and other high-tech home care skills preferred
Responsibilities
- Assess patient and caregiver needs using the nursing process to develop individualized care plans
- Serve as case manager for assigned patients and coordinate home health visits efficiently
- Complete accurate and timely OASIS documentation and other required clinical records
- Implement nursing interventions, treatments, and medical plans of care in the home setting
- Provide patient and caregiver education on medications, treatment options, discharge instructions, and reportable conditions
- Coordinate care with physicians, interdisciplinary team members, vendors, and support services
- Perform high-tech nursing tasks as needed, including IV care, wound care, and wound VAC management
- Support safe transitions, discharges, and transfers while maintaining compliance with agency and regulatory standards
- Participate in on-call coverage, weekend rotation, and holiday assignments as scheduled
- Advocate for patients and families to ensure quality, compassionate, and timely care
