• Schedule: Monday-Friday, 8:00 AM-4:00 PM
• Shift Duration: 8-hour days
• Additional Coverage: On-call, weekend rotation, and holidays required
• Weekly Estimated Pay: $2,500-$2,630

Job Requirements

• Active Registered Nurse (RN) license
• At least 1 year of nursing experience required; home health or hospice RN experience strongly preferred
• OASIS documentation experience required
• Experience with case management and patient-centered care planning
• Strong knowledge of home health workflows, clinical documentation standards, and compliance requirements
• Comfortable working independently in the field and managing a patient caseload
• Ability to support on-call hospice coverage as needed
• Experience with wound care, IV therapy, wound VACs, and other high-tech home care skills preferred

Responsibilities

• Assess patient and caregiver needs using the nursing process to develop individualized care plans
• Serve as case manager for assigned patients and coordinate home health visits efficiently
• Complete accurate and timely OASIS documentation and other required clinical records
• Implement nursing interventions, treatments, and medical plans of care in the home setting
• Provide patient and caregiver education on medications, treatment options, discharge instructions, and reportable conditions
• Coordinate care with physicians, interdisciplinary team members, vendors, and support services
• Perform high-tech nursing tasks as needed, including IV care, wound care, and wound VAC management
• Support safe transitions, discharges, and transfers while maintaining compliance with agency and regulatory standards
• Participate in on-call coverage, weekend rotation, and holiday assignments as scheduled
• Advocate for patients and families to ensure quality, compassionate, and timely care
