Job Requirements

  • Active Registered Nurse (RN) license
  • Minimum of 1 year of bedside clinical experience required; LTAC or acute care experience preferred
  • BLS certification required before first shift
  • ACLS certification required before first shift
  • Ability to provide safe, compassionate, and organized patient care
  • Strong communication, teamwork, documentation, and critical thinking skills

Responsibilities

  • Perform nursing assessments within 8 hours of admission and use findings to support an individualized plan of care
  • Coordinate with patients, families, and the interdisciplinary care team to identify teaching needs and care goals
  • Document assessments, interventions, and patient responses accurately and in a timely manner
  • Support the rehabilitation program and deliver direct patient care using the nursing process
  • Initiate discharge planning from admission and reinforce patient education throughout the stay
  • Communicate changes in patient condition clearly to physicians, charge nurses, and other healthcare team members
  • Provide guidance and supervision to ancillary nursing personnel as needed
  • Promote patient and employee safety while following facility policies and best practices
  • Contribute to a positive, efficient unit culture through teamwork, professionalism, and patient-centered care
  • Take on charge responsibilities after orientation when requested
