- Schedule: Monday through Friday, 8:00 AM to 5:00 PM
- Weekly Estimated Pay: $1,880 to $1,980
- Patient Volume: Approximately 5–8 patients per day
- Productivity Expectation: 30 points per week
- Travel Territory: Up to 75 miles from the assigned care center, including surrounding local communities
- Weekend/On-Call: Weekend rotation and on-call coverage may vary by assignment
- Documentation System: HomeCare HomeBase; OASIS proficiency required for RNs
Job Requirements
- Current Registered Nurse (RN) license in hand
- Minimum 1 year of adult home health experience or 1 year of hospice experience, depending on specialty
- Recent experience with HomeCare HomeBase within the last 6 months preferred
- OASIS proficiency required for RNs
- Current CPR/BLS certification
- Reliable transportation, valid driver’s license, and auto insurance
- Willingness to travel daily to patient homes across the assigned service area
- Comfortable working independently and managing a full home health caseload
- Ability to complete documentation within 24 hours of each visit
- Open to on-call rotations and weekend coverage as needed
Responsibilities
- Provide skilled nursing care in the home setting based on physician orders and care plans
- Complete start-of-care, wound care, follow-up visits, and other home health visits as assigned
- Assess patient needs, implement nursing interventions, and update care plans accordingly
- Educate patients and families on treatment plans, safety, and self-care techniques
- Coordinate care with the interdisciplinary team to support positive patient outcomes
- Supervise and evaluate home health aides and LPN/LVNs when applicable
- Travel between patient homes within the assigned territory and attend required meetings
- Document all care thoroughly and accurately in the EMR within required timelines
- Maintain compliance with productivity standards and assignment expectations
- Deliver compassionate, high-quality care that helps patients remain healthy at home
