Job Details

• Position: Registered Nurse (RN) – Home Health
• Setting: In-person home health / community-based care
• Location: Grand Junction, CO 81506
• Start Date: 09/08/2026
• Duration: 13 weeks
• Shift: Monday-Friday day schedule with on-call rotation
• Shift Duration: 12-hour rotating
• Weekly Estimated Pay: $1,680-$1,770
• Guaranteed Hours: 40 per week

Job Requirements

• Active Registered Nurse (RN) license in Colorado or an active NLC compact license
• Home health experience preferred
• Valid driver's license, reliable transportation, and auto insurance
• Strong assessment skills and the ability to work independently in a community-based setting
• Excellent critical thinking, organization, and time-management skills
• Clear, professional communication with patients, families, and interdisciplinary team members
• Comfort with documentation and EMR systems; HCHB experience preferred

Responsibilities

• Perform comprehensive patient assessments in the home setting
• Develop and update individualized plans of care
• Provide skilled nursing care and appropriate interventions
• Monitor patient progress and report changes to the care team
• Educate patients and families on medications, treatments, and care plans
• Coordinate care with LPNs, therapists, caregivers, and providers
• Complete accurate and timely documentation in the EMR
• Participate in the scheduled on-call rotation
