Job Requirements
- Active Registered Nurse license in the State of Michigan
- Current Basic Life Support (BLS-HCP) certification
- Certified Diabetes Care and Education Specialist (CDCES) or Certified Diabetes Educator (CDE) credential
- At least 1 year of relevant nursing or diabetes education experience preferred
- Strong critical thinking skills and the ability to work independently with minimal supervision
- Comfortable working in a fast-paced outpatient clinical environment and taking appropriate action when needed
- Ability to communicate clearly with patients, families, and interdisciplinary care team members
Responsibilities
- Provide diabetes education, consultation, and evaluation of patient and program outcomes
- Create, communicate, and implement individualized teaching plans for patients, families, and support persons
- Deliver patient education in both group and one-on-one settings using a variety of teaching methods
- Adapt educational approaches to meet the developmental and clinical needs of patients across the lifespan
- Support maintenance of diabetes supplies, educational materials, and patient resources
- Assist other clinical staff as needed to ensure smooth daily workflow and quality outpatient care
- Participate in orientation, training, meetings, and reporting as assigned
