Travel Home Health & Hospice Registered Nurse (RN) – Cordova / Memphis, TN

• Shift Duration: 8-hour shifts
• Schedule: Monday through Friday, 8:00 AM to 4:30 PM
• On Call / Weekends: None
• Coverage Areas: Shelby, Fayette, and Tipton counties
• Weekly Estimated Pay: $2,120 - $2,230
• Openings: 2
• Mileage Reimbursement: Available between visits, from first patient to last patient
• Productivity Expectation: 6 visits per day / 30 points per week

Registered Nurse Job Requirements

• Current, active RN license
• Minimum of 1 year of nursing experience required
• Hospice experience required
• Hands-on experience with HCHB (Homecare Homebase) required
• Associate degree minimum
• Strong communication skills and a professional, patient-centered approach
• Flexible, easy-going, and pleasant personality

Responsibilities

• Perform case management for hospice patients in the home setting
• Complete admissions and support a smooth transition into care
• Prepare for interdisciplinary team meetings and coordinate with the care team
• Deliver compassionate, high-quality nursing care during patient visits
• Develop, update, and communicate care plans based on patient needs
• Document accurately in HCHB and maintain productivity expectations
• Provide support across assigned coverage areas while representing the highest standards of home health care and hospice nursing
