Job Requirements
- Active Registered Nurse (RN) license
- Minimum of 2 years of bedside nursing experience
- BLS certification required prior to first shift
- ACLS certification required prior to first shift
- Strong assessment, documentation, and communication skills
- Ability to work effectively in a fast-paced acute care or LTAC environment
- Experience with rehabilitation planning and discharge coordination preferred
Responsibilities
- Complete timely nursing assessments and develop individualized plans of care
- Provide direct patient care using the nursing process of assessment, planning, implementation, and evaluation
- Collaborate with patients, families, physicians, and the interdisciplinary care team to support patient outcomes
- Document assessments, interventions, and patient responses accurately and in a timely manner
- Support patient rehabilitation efforts and participate in weekly care conferences
- Initiate discharge planning and patient education from admission through discharge
- Communicate changes in patient condition promptly to promote continuity of care and patient safety
- Assist with supervision and direction of ancillary nursing personnel as needed
- Promote a safe, organized, and positive environment for patients and staff
