Job Requirements

• Active Oregon RN license or compact license on file
• Minimum 1 year of case management or Patient Care Coordinator experience required
• BLS certification required
• EPIC charting experience preferred
• Experience working with Medicaid and Medicare populations preferred
• Previous experience in healthcare coordination, outpatient support, or clinical support settings is a plus
• Ability to work weekends and holidays as needed
• Willingness to float to nearby locations when required

Responsibilities

• Coordinate patient care needs and support continuity of services across the care team
• Communicate effectively with patients, families, and clinical staff to ensure smooth care transitions
• Document accurately in EPIC and maintain timely, organized records
• Support scheduling, follow-up, and care coordination efforts for assigned patients
• Assist patients covered by Medicaid and Medicare, ensuring appropriate coordination and resource support
• Collaborate with interdisciplinary team members to address patient needs and promote positive outcomes
• Float to neighboring locations as needed to support operational needs
• Provide dependable, patient-centered support in a fast-paced healthcare environment
