• Contract Duration: 13 weeks
• Schedule: 40 hours per week, Monday through Friday, with occasional weekends and holidays
• Estimated Weekly Pay: $2,250 - $2,370
• Guaranteed Hours: 40

Job Requirements

• Current Massachusetts Registered Nurse (RN) license required
• At least 1 year of nursing experience required; 6 months of nursing experience may be considered with a BSN
• CPR certification required
• Successful completion of a pre-hire medication test
• Home care, home health, or IV nursing experience preferred
• BSN preferred
• Strong computer skills and accurate clinical documentation abilities

Responsibilities

• Complete comprehensive nursing assessments at admission, including physical, psychosocial, environmental, and health history factors
• Develop, implement, and revise individualized plans of care based on patient needs and clinical findings
• Use clinical judgment to establish visit frequency and coordinate interdisciplinary home health care
• Monitor patient progress, evaluate outcomes, and adjust care plans as needed
• Provide education and documented supervision of the Home Health Aide plan of care every 14 days
• Communicate significant changes in patient condition to the physician and other involved care providers
• Complete admissions, charting, discharge summaries, and related paperwork in a timely manner
• Obtain and clarify valid physician orders for all care delivered
• Coordinate safe and appropriate patient discharges
• Support case conferences, community resource referrals, and additional duties as assigned
