Registered Nurse Job Requirements

• Active Registered Nurse (RN) license
• Minimum Two (2) Years Bedside Clinical Experience highly preferred
• BLS certification required prior to first shift
• ACLS certification required prior to first shift
• Strong assessment, documentation, and communication skills
• Ability to work effectively in a collaborative, patient-centered care setting
• Experience in long-term acute care, rehabilitation, or post-acute care preferred

LTAC Nursing Responsibilities

• Perform comprehensive nursing assessments within 8 hours of admission
• Develop individualized plans of care in collaboration with patients, families, and the interdisciplinary healthcare team
• Provide timely updates for team conferences and patient re-evaluations
• Document assessments, interventions, and patient responses accurately and promptly in the medical record
• Support patient rehabilitation efforts and promote positive outcomes in an LTAC setting
• Initiate discharge planning and patient education from day one
• Communicate changes in patient condition to the appropriate care team members
• Delegate and supervise ancillary nursing personnel appropriately
• Maintain a safe, organized, and efficient clinical environment
• Contribute to a positive unit culture through teamwork, professionalism, and patient advocacy
