Job Requirements

• Active Registered Nurse (RN) license
• Current CPR certification
• Minimum of 1 year of nursing experience preferred
• Home health experience and HCHB proficiency required
• Associate degree in Nursing or higher
• Strong communication, organization, and time-management skills
• Ability to work independently in a home health environment
• Comfort providing field nursing care across the Waldorf, MD area

Responsibilities

• Complete skilled nursing visits in patients’ homes
• Perform admissions, discharges, and follow-up visits
• Assess patient needs and deliver high-quality clinical care
• Document visits accurately and in a timely manner
• Coordinate care and communicate effectively with the interdisciplinary care team
• Provide patient and family education as needed
• Maintain a professional appearance and follow home health care standards in the field
