• Weekly Hours: 40 hours per week
• Shift Duration: 8-hour shifts
• Schedule: Monday through Friday, approximately 8:00 AM to 4:30 PM
• Weekend Requirement: 1 weekend per month minimum with LPN backup
• Estimated Weekly Pay: $2,120 - $2,230
• Patient Care Focus: Home health and hospice practice
• Coverage Area: Regional home care coverage within nearby counties

Job Requirements

• Current RN license in Tennessee
• Current BLS certification
• Associate degree required
• Minimum 3 years of RN experience
• At least 1 year of home health RN experience
• Strong clinical judgment and the ability to work independently in the home setting
• Solid documentation, communication, and patient education skills
• Ability to work in solid-colored scrubs

Responsibilities

• Deliver skilled nursing care to patients in the home setting
• Perform nursing procedures to support treatment and rehabilitation plans
• Assess patient needs and help ensure psychosocial needs, safety, and comfort are met
• Monitor, document, and report patient status and changes in condition
• Teach patients, families, and support personnel on proper health maintenance and care practices
• Evaluate care outcomes and collaborate with the care team to support optimal results
• Support adherence to the nursing process and individualized care plans
• Participate in weekend coverage rotation as scheduled
