• Weekly Estimated Pay: $2,250-$2,370

Job Requirements

• Active Massachusetts RN license
• Minimum of 1 year of nursing experience required, or 6 months of nursing experience with a BSN
• Current CPR certification required
• Successful completion of the pre-hire medication test
• Home health, home care, and IV experience preferred
• Strong computer skills preferred
• BSN preferred
• Ability to provide high-quality care independently while maintaining excellent documentation standards

Responsibilities

• Complete comprehensive nursing assessments upon admission, including physiological, psychosocial, environmental, and health history factors
• Develop, implement, and revise individualized plans of care based on patient needs
• Maintain accurate and timely documentation, including narrative notes, admission paperwork, and discharge summaries
• Coordinate care with physicians, interdisciplinary team members, and community resources as appropriate
• Monitor patient progress and communicate significant changes in condition to the appropriate care providers
• Provide and document supervision of the Home Health Aide plan of care every 14 days
• Obtain and clarify valid provider orders for all care delivered
• Support discharge planning and ensure all required discharge paperwork is completed appropriately
• Participate in interdisciplinary case conferences and contribute to effective care planning
• Perform all other duties as assigned in support of safe, skilled home care
