• Schedule: Monday-Friday, 8:00 AM to 5:00 PM
• Shift Duration: 8-hour days
• Hours Per Week: 40
• Weekend Coverage: Some weekend coverage required
• Call: Occasional emergency-based call may be scheduled
• Weekly Estimated Pay: $2,000-$2,100

Qualifications

• Active Registered Nurse (RN) license and current BLS certification required
• Minimum 1 year of experience in home health, hospice, or related nursing care preferred
• OASIS experience required
• Experience with travel nursing or traveling between patient homes preferred
• Valid driver’s license in good standing
• Experience using HealthWyse (Careficient) EMR preferred
• Strong clinical judgment, communication skills, and a patient-centered approach
• Ability to stay organized and adapt to changing schedules in a mobile care environment

Responsibilities

• Provide skilled nursing care to patients in home health and hospice settings
• Complete accurate assessments, documentation, and care coordination using OASIS guidelines
• Deliver compassionate, high-quality outpatient care tailored to individual patient needs
• Travel to patient homes and adapt to daily schedule changes as needed
• Support wound care and other assigned homecare nursing services
• Collaborate with the interdisciplinary care team to promote safe, effective patient outcomes
• Respond appropriately to urgent needs when emergency call coverage is assigned
