Job Requirements

• Active Registered Nurse license in West Virginia
• Home health experience
• Experience with HCHB and OASIS
• Ability to work independently while collaborating with interdisciplinary care teams
• Strong documentation, communication, and organizational skills
• Comfortable with on-call responsibilities and occasional weekend coverage

Responsibilities

• Assess patient health status and implement individualized care plans
• Provide skilled nursing care for well, chronic, or acutely ill individuals in home and community settings
• Monitor medications, ensure accurate documentation, and support safe medication practices
• Complete daily, monthly, quarterly, and annual nursing documentation as required
• Communicate changes in patient condition to physicians and other members of the care team
• Coordinate care with providers, discharge teams, and other healthcare professionals
• Support staff education and training related to health, safety, and disease prevention
• Help ensure supplies, equipment, and furnishings are maintained to support a safe care environment
